The Feet Team accepts patients referred to by their GP with a Enhanced Primary Care plan (EPC) and Complex Disease Management plan (CDM). It is a great way to ensure a multidisciplinary, team based care approach between your health practitioners. Individuals are entitled to the rebate of 5 visits per calendar year if you are deemed eligible by your general practitioner. This rebate is $60.30 with an out of pocket expense, depending on your appointment type. 

To be eligible for the government funded initiative, you must have a chronic medical condition which lasts longer than three months and your general practitioner must be assisting you with this condition.

• Osteoarthritis 

• Musculoskeletal Conditions 

• Diabetes 

• Heart Attack 

• Stroke 

• Cardiovascular Disease 

• Asthma 

• Cancer 

• Autoimmune Disease

Department of Veteran Affairs (DVA) Gold health card holders are able to receive free podiatry treatment at The Feet Team with a referral from the GP (D904). With a referral form from your GP you are eligible to 12 months of free podiatry or 12 appointments whichever one is reached first. 

Services available are: 

• Routine foot care

• diagnostic - vascular, neurological, muscle testing and gait analysis. 

• Orthotics or medical grade footwear if required.
